Frequently Asked Questions

To request services as a new client, please fill out the intake request form. You may also contact me by phone, 425-484-0790, if you prefer. I will respond to you within 2-3 business days to coordinate services. Prior to your first scheduled appointment, you will receive an email from the Five Oaks Counseling secure electronic records system with a link to review and complete all required intake forms.

Confidential conversations between a therapist and a client are something that therapists are bound to by the ethical standards of their profession. The purpose of confidentiality is to provide a safe environment where a client will freely discuss problems, without fear that very personal information will be revealed to others. Both office and on-line sessions are conducted in an office with a closed door.  Client files are stored behind two locks per HIPPA regulations. Any other disclosure of information about one's therapy requires written permission.

I am an out of network provider with all insurance companies. Full payment is due at the time of service. I will provide you with a receipt/superbill which includes the appropriate codes and payments. You can submit the paperwork to your insurance company for reimbursement.

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

Some recommended questions to your insurance provider to help determine your benefits are:

  • Does my health insurance plan include mental health benefits?

  • Do I have a deductible? If so, what is it and have I met it yet?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • Do I need written approval from my primary care physician in order for services to be covered?

*Please contact me regarding sliding scale availability. I believe everyone should have access to affordable counseling.
